PERSYARATAN USUL MENJADI PNS

1. Pengantar dari Pimpinan Unit Kerja ;
2. Fotocopy sah SK CPNS ;
3. Fotocopy sah Ijazah terakhir ;
4. Fotocopy sah Surat Pernyataan Melaksanakan Tugas (SPMT) ;
5. Fotocopy sah Surat Tanda Tamat Pendidikan Pelatihan (STTPP) Prajabatan ;
6. Fotocopy sah DP-3 (Jangka Waktu Penilaian 1 tahun terhitung dari SPMT) ;
7. Asli dan Fotocopy sah Surat Keterangan dari Tim Penguji Kesehatan
(RSUD H. Abdul Aziz Marabahan).

Semua persyaratan (sesuai urutan diatas) dimasukkan dalam map jepit
(snelhecter map), warna :
 Merah untuk Tenaga Guru
 Hijau untuk Tenaga Kesehatan
 Biru untuk Tenaga Teknis

Di depan Map ditulis : Nama, NIP, Unit Kerja dan Nomor Handphone
Masing-masing dibuat dalam rangkap 2 (dua).
Berkas usulan kami terima paling lambat tanggal 1 NOPEMBER 2010.
